The first reach for your shirt may wake the ache. Dressing can hurt, and sleep may have been poor again.

Arthritis, a worn tendon, or an old injury may be involved. I'd start with the day you noticed it and the movement that brings it on.

## What to notice during the day

Pain on top of the shoulder often shows up while reaching. A deeper ache may come with stiffness and less turning.

Notice whether your arm feels weak or simply hurts when lifted. Also note any fall, new chore, or change in sleep.

Let the exam come before any label. These details won't solve the soreness, but they'll give the doctor useful facts.

## What to try while it calms down

Let the shoulder move gently within a comfortable range. Pause any chore that brings on a sharp ache.

Warm water may loosen the joint when morning stiffness hangs on. Wrapped cold won't loosen stiffness, but it may soothe soreness after heavier use.

Try sleeping on the other side with the sore arm supported. Cut back when yesterday's movement still hurts after waking.

## What to ask if it doesn't settle

A visit can check motion, strength, sore areas, and your neck. A scan may come up if the result would change your care.

QC Kinetix calls care made from your blood regenerative treatments. Clinic staff take blood from your arm, spin it so platelets—the blood parts carrying growth factors—collect in the liquid, then place that liquid near your sore shoulder with a needle.

PRP stands for platelet-rich plasma and uses that prepared blood liquid. Concentrated PRP is made the same way but leaves still more platelets in it.

Medical providers—the doctors and nurses who examine you—review your health and past care first, because medicines, old injuries, and the exam can all affect whether this care fits. They'll explain whether extra platelets change the likely benefit, cost, or recovery for your shoulder.

Joint preservation is care meant to keep your shoulder working and delay surgery. Ask what better quality of life would mean for sleep, dressing, driving, or reaching.

These non-surgical choices won't fit every sore shoulder. Take current medicines, earlier scan reports, and a short account of past care.

## What Queen Creek activity can tell you

Pickleball, riding, yard work, and cycling ask different things of a shoulder. Note the exact reach, lift, pull, or fall that came before your soreness.

Most easy activity can continue while you sort it out. Ease the task that hurts, and get help quickly after major trauma or sudden weakness.

For a scheduled visit, QC Kinetix (Chandler) is at 1100 S. Dobson Rd., Suite 210, Chandler, AZ 85286. From central Queen Creek, travel west on Ocotillo toward Loop 202 and the Dobson Road exit; SR 24 connects the northeast with Loop 202.

Call (602) 837-PAIN / (602) 837-7246 before traveling. Banner Ironwood on Gantzel Road is for emergency symptoms, not a routine clinic appointment.

1. A meta-analysis of 14 placebo cohorts from 13 Level-1 knee OA injection trials (1,076 patients, KL 1-4) measured what the SALINE arms did. Intra-articular normal saline produced a statistically significant VAS pain improvement at 3 months (MD 12.10) and a larger one at 6 months (MD 16.62), reaching clinically meaningful thresholds. Any uncontrolled report of 'my injection worked' has to clear this bar before it means anything about the injectate.

2. A network meta-analysis in Annals of Internal Medicine showed that placebo treatments in osteoarthritis trials are not equivalent to one another: the delivery route itself changes the response, with intra-articular placebo producing larger effects than oral placebo. This is a structural reason to distrust any comparison of an injected product against a pill, or against no treatment.

3. The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

4. A meta-analysis of eight randomized trials of PRP for rotator cuff tendinopathy concluded PRP was a safe and effective intervention for long-term pain control and shoulder function - but the review itself records that PRP preparation and injection technique varied between studies and that the control interventions differed (saline in four trials, rehabilitation or dry needling in the other four), which is why its conclusion sits alongside more cautious reviews rather than settling the question.
